I found in my files a PDF that I saved from the work of John M. Purcell on 
Ohio municipal flags from his paper "Civic Flags of Ohio's Smaller Cities" from 
the XIV Congres Internacional de Vexil-logia Barcelona 1991.
"The flag of 
the City of Richmond Heights utilizes the swallow-tail design, typical of many 
Ohio civic flags, ostensibly in imitation of the shape of the state's flag.The 
field of the flag is blue, with a white V-shaped triangle issuing from the 
center of the hoist. The flag was designed by Carol Ann Bigus, a secondary 
school student in 1969."

A quick Google image search finds that the flag still exists and is being 
used by the city.
